Purpose of the position:
The incumbent will provide comprehensive HIV and TB clinical services, an integrated cascade of care, inclusive of diagnosing, treating, monitoring and evaluating communicable and infectious diseases. They will be responsible for the delivery of HIV and TB clinical services to patients at primary health care clinics in compliance with the project’s HIV and TB service profile and reporting requirements.

They will work collaboratively with outreach teams and contracted community-based organisations (CBOs) in monitoring the adherence of decanted patients, e.g. reporting non-collection of medicine parcels at Central Chronic Medicine Dispensing and Distribution (CCMDD) pick-up points.
Duties and Responsibilities
- Provide services using evidence-based HIV and TB interventions in all streams according to the DoH policy, guidelines and SOPs.
- Optimise HTS at facilities
- Implement the Universal Test and Treat (UTT) policy according to the latest guidelines to support attainment of UNAIDS 95-95-95 targets
- Monitoring referrals and ensuring successful referrals are validated
- Support the Ideal Clinic Realisation and Maintenance (ICRM) programme and quality improvement interventions at the facility
- Implement an effective appointment system and differentiated care models of the decanting programme
- Promote patient decanting of stable patients by creating demand for the CCMDD service and TLD transition and identifying eligible patients
- Participate in facility QIP using bottle-neck analysis
- Keep abreast with new clinical developments e.g. training, in-service training, reading journals, policies, and procedures related to job
- Implement case management and support patient retention in HIV and TB programme
- Participate in COVID-19 screening, referral and ensure compliance with infection control measures
- Record and trace patients interrupting treatment working with coaches and case officers and verify lists of ‘not confirmed as linked to care’
- Conduct weekly reviews to clear ART waiting patient lists and trace clients (adult and paediatric) for treatment initiation
- Review cohort reports and facilitate tracing of PCR-positive babies and HIV positive women for treatment initiation
- Monitor viral load uptake and suppression using eLabs
- Develop an effective system to monitor viral load tests done on a weekly and monthly basis and results for action (rFA)
- Scale up CCMDD patient registration and prescription on SyNCH
- Participate in data analytics

Minimum Requirements
- Diploma in Nursing or Bachelor's degree (B. Cur) in Nursing
- Valid SANC registration
- NIMART trained and certified
- Adult Primary Care (APC) is essential
- 2 years’ experience in nursing post-registration
- 2 years’ experience in HIV and TB programmes
- Experience with NIMART, CCMDD, Sexual Reproductive Health (SRH), Maternal and Child Health (MCH)
- Dispensing licence will be an added advantage
- Willingness to travel as and when required
- Proficiency in Microsoft Office package
- Ability to communicate in IsiXhosa and English or another South African language
- 3 years’ experience in Primary Health Care programme
- Valid code 08 driver’s licence
- Clear criminal record
